Mr. Brady EMT Course Agreement Forms
In order for your student to participate in this course, it is required that you as the parent/guardian read, review, and agree to the legal documents listed below. Please take the time to discuss these documents with your student so that they are fully aware of the rules, regulations, and procedures that will help them be successful. Please contact Mr. Brady at 435-527-4651 or scott.brady@seviersd.org if you have any questions, comments, or concerns regarding these materials. Printed copies will be made available per your request.
Your student will NOT be able to start the course until these forms are signed.
Please click on each agreement below and review with your student prior to signing. When you have reviewed all four documents, click on the green "Take Survey" button below and complete the questions.
In addition to these forms, students and parents/guardians will be required to complete the following EMT-specific forms (to be completed and signed electronically):
Finally, students and parents/guardians will be required to sign an Sevier County (Under 18) waiver in order to ride as a trainee on the ambulance. This will be handed out on paper.
